1. Call us to schedule your new patient exam. You can reach us at 269-372-1042
2. Complete your Paperwork
- Fill out the New Patient Registration AND Medical History Forms at least 2 weeks before your appointment. If not received, we will be unable to continue to reserve your appointment.
3. Transfer Dental Records from your previous office (if applicable)
- If records are not received prior to your new patient exam, we will need to take new x-rays (note: insurance may not cover duplicate x-rays).

What to Expect at Your First Visit
Your first visit to Oshtemo Family Dentistry is an important one as it introduces you to our practice and our wonderful team. During the first visit, our doctors will get to know you and complete a comprehensive dental exam. This visit allows us to assess your oral health, take any necessary X-rays, and create a customized treatment and hygiene plan before scheduling your first cleaning.
